For Physician Referrals
Your physician can refer you to the Fixel Institute by completing the Patient Appointment Request Form.

Please complete the form in its entirety and fax it to 352-627-4295 along with all applicable clinical notes, recent lab work, radiology reports, copies of the front and back of the patient’s insurance card(s) and any other pertinent medical records.
